At Mayfair Diagnostics, we have 14 medical imaging clinics across Calgary. We also have one each in Cochrane, Saskatoon, and Regina. To make it easier for our patients to access the most up-to-date information about available appointment times across all of our locations and to effectively handle the volume of requests, the majority of our appointment bookings are handled in one central location by our Customer Contact Centre.

In addition to booking requests, our booking team handles numerous requests for information, such as exam preparation instructions, directions, and parking instructions.
We collected the most-asked questions from patients while booking their appointment. Below are the questions and their answers.
Your requisition is the paperwork that documents your doctor’s request for a specific exam or treatment and explains why it’s needed. In some cases, your doctor may have already faxed the requisition to us, but often you will need it to let us know which exam your doctor requested to book an appointment. For many exams, the College of Physicians and Surgeons of Alberta (CPSA) and College of Physicians and Surgeons of Saskatchewan (CPSS) does not allow us to proceed without a requisition. However, there are exceptions, such as self-referred mammograms or follow-up breast imaging when we contact you directly.
If your doctor gives you a requisition from another provider but you have a preferred Mayfair clinic that’s more convenient for you, any of our clinics will accept that requisition.
*Please note, the referring doctor listed on the requisition must be currently practicing, and the referral must originate from within Canada.
If your doctor faxes a requisition to us, the contact centre will call you to book your appointment. When requests for medical imaging are sent to the Mayfair Diagnostics Customer Contact Centre, each requisition is reviewed by a member of the team, and a radiologist if necessary. These forms are checked for accuracy, appropriateness, availability, and any special considerations. For more information about medical imaging requisitions, please refer to this article.
After each requisition is reviewed, a booking coordinator will then contact you. Depending on the complexity of the exams being requested, this may take from 2-3 days after receiving the requisition from your referring doctor. Urgent requests are handled immediately.
Mayfair Diagnostics offers both email and text message notifications about your medical imaging appointment. These notifications will include a confirmation of your appointment with a link to exam and clinic details, as well as reminders prior to your appointment.
*Please note that for privacy reasons, you will need to click the link in your email or text message to login to our Patient Connection system to see your exam preparation instructions. Alternatively, you can look up the preparation instructions on our website.
The first time you book with us, you will be asked if you would like to receive these notifications and, if you agree, to provide your cell phone number or email address. Depending on your preference, you will receive either text or email notifications about your appointment. You may opt out for either notification by replying STOP to the text notification or clicking the unsubscribe option within the email.
Through these email and text notifications, you will also be able to digitally confirm your appointment, or request to reschedule or cancel booked appointments. This option allows you to use text or email to make these requests without needing to call us.
We ask you to bring your photo identification, your Alberta Health Card and requisition to every visit. Your requisition is the paperwork which documents your doctors request for a specific treatment or exam and also explains why they are needed. If your doctor has already faxed your requisition to us and it’s already in our system, then you don’t need to bring it to the appointment. If your doctor has not sent your requisition to us, you will need to bring it to your appointment, as we need to scan a copy of it for when you check-in.
If you require an assistive support person to help with communication, mobility, or medical needs during your appointment, you may bring one individual to accompany you. We request that you do not bring children who require supervision to your appointment. For their safety, children are not allowed in the exam room and must always be supervised in the waiting area. If you need a translator, you should bring one support person with you to assist with communication. For obstetrical ultrasound appointments, you may bring one person to accompany you into the exam room.
*Please note the person accompanying you for any of these needs must be 16 years of age or older.
