Before booking online or calling us, ensure you have your provincial health card and your requisition form available.
You can book, request, reschedule, or cancel an appointment using our new online booking tool, Patient Connection. To protect your health information, a mobile
number is needed for multi-factor authentication.
Patient Connection is currently only available to existing Mayfair patients. New
to Mayfair? Call our Contact Centre to book your first visit, then use Patient
Connection next time.
We offer centralized booking for appointments at all our clinics, and we accept requisitions from any Canadian medical imaging company. Visit our locations page for information on specific clinics.
